Directions

  1. Please type the background information in the order given.
  2. Type the application narrative using no more than 2 pages, single spaced, one side.
  3. Include all aspects of the school library media project requested in the application narrative. Do not send pictures, clippings, scrapbooks, or handouts unless requested later.
  4. All application materials must be received in the MASL Office no later than close of business November 15 (6:00 p.m. CT).
    Any applications received after the stated deadline will not be reviewed.
  5. The Awards Committee may request additional information about the project to determine a winner.

BACKGROUND INFORMATION

  1. List the following information in the order given:
  2. Title of project and a brief statement of its purpose, including target group and expectations.
  3. Name of nominee, title, home and work phone numbers, email address, fax number.
  4. Official name of district, superintendent, school, building principal, school address, and phone number.

APPLICATION NARRATIVE

Write a narrative for each section. The application narrative may not be more than 2 pages, single-spaced, one side.

Section 1 - Project Description and Execution (75 points)

  1. Specifically describe the project, making sure the following are included:
    • Level and number of students affected
    • Duration of project (ongoing, periodic, annual)
    • Method of implementation (materials, techniques, personnel used)
    • Other library and/or community involvement, if applicable
    • Personnel responsible for implementation (LMS, clerk, volunteers, other)
  2. How is this project innovative and creative or an improvement in comparison with similar library media center projects?
  3. Give specific examples of how this project has made a difference to students and staff members.

Section 2 - Leadership (15 points)

Describe the library media specialist's role in the formulation and implementation of this project and in its application to the primary audience.

Section 3 - Contribution to the Educational Environment (10 points)

What parts of the project are unique to the school, and what parts could be utilized by other school media specialists with little or no adaptation?
